The following information is from the Health Care Worker (HCW) program of the Rogue Valley Council of Governments Senior and Disability Services. It is a list of persons who have been authorized to receive payment by Senior and Disability Services for services provided to clients. The requirements for acceptance are minimal; it only includes a criminal records check for conviction of a serious crime within the last five years and attending a short orientation on the rules of the Client Employed Provider program. The persons listed are not employees of the State of Oregon or RVCOG and are considered employees of individual clients. Senior and Disability Services is not responsible for the quality of work performed or other actions by a provider hired privately. We provide this public information as a service to potential employers and those who are seeking this type of employments. You are strongly encouraged to thoroughly interview and request references of any caregiver you are considering hiring.

If you need assistance in your home (for example, meal preparation, assistance with bathing, housekeeping) and are unable to pay for the services privately, please contact us to ask about the programs available. The goal of the Health Care Worker program is to make available quality care that supports an individual’s choice to remain in their home with assistance in the areas of personal care and housekeeping. We have several types of services for qualified individuals. Please call for individualized information at one of  the numbers given above.

The list is maintained by the Health Care Worker Clerk in each Branch Office. If you have questions, please contact the HCW clerk in the appropriate office at the above numbers.

To get a list of all Health Care Workers looking for work in your county, select county and then select detailed list or less detailed list. To get a list of Health Care Worker who live in your area, are willing to work in special situations (for example, smoking), work a specific schedule and/or do specific tasks (for example, bathing), select these options, then select detailed list or less detailed list.
